When there is no inhibition of peristalsis, the distention only gives rise to discomfort, and as the affection becomes localized the abdomen usually assumes its natural appearance. With the beginning of spreading peritonitis the abdomen is often flat, and even scaphoid.
Distention usually occurs early, however, and may be extreme, the abdomen being dome-shaped and perfectly motionless. In cases of distention due to profoimd infection, no sound whatever is heard on auscultation. Rich- ardson has
... observed that in cases of great distention there is also at times a serious interference with the portal circulation, the distended intestine being dark red or purple, and the portal radicles dark and prominent.
In these cases the heavy distended coils can be felt through the abdominal wall. Portal thrombosis may give rise to a similar condition. Marked distention may also be occasionally observed in cases of profound toxaemia, without any evidence of mechanical obstruction or peritonitis. Extreme distention is one of the gravest symptoms observed in appendicitis, whether due to a local infection, to mechanical obstruction, or to general intoxica- tion.
