It is a subject for deep consideration, and one which at times per- plexes the most astute and experienced surgeon. I cannot consider, except in a brief manner, some points which, by experience, have proven to be paramount.
In conditions involving the stomach, such as perforation by a gastric ulcer, the condition becomes acute only so far as the effect upon the abdominal cavity is concerned. Patients suf- fering from gastric ulcer give sufficient reason for diagnosis and attention long before a... perforation takes place. When a perforation occurs the time to operate is just as soon as the diagnosis is made and the patient can be put in the hospital or suitable place for safe surgical interference. It is not within the province of the subject allotted me to go into the diagnostic points of these cases.
In acute gall bladder infections, where the cystic or common ducts are not occluded, it is best to defer any surgical interfer- ence until an effort, by rest in bed, has given the inflamma- tory condition time to subside.
